Requirements for Application

1. Scanned copy or photo of Grade 11 Report Card (in PDF File)

• Must show all grades and the general weighted average of the student.
• If the Grade 11 report card is not yet complete, this document must at least reflect all the subjects for 1^st Semester and 2^nd Semester. Note that the initial report card will be used for assessment only. Final deliberation will only take place once the complete Grade 11 Report Card is submitted.
• All pages of the report card must be saved in one .pdf file
• The file must not exceed 5MB

2. ID Picture

• Must draw the full face until chest of the applicant, properly groomed, and in decent attire 
• With white or any plain, light colored background 
• Does not contain other people or objects other than the applicant 
• The file must not exceed 5 MB  

3. Application fee of P200 (inclusive of service fees)

• Pay via DragonPay.
• Click the button on the page that says Application Fee (S.Y. 2022-2023).

4. Active Gmail account

• Note that it is preferred to have your First Name and Last Name on this account. Remember and take note of your password. All official communication of the school regarding your application will be sent to this email address. 

Guide for Online Registration

1. Apply online by visiting this link:

2. Select “Grade 12” in the intended Grade Level Portion

3. Fill out the application form. Use a Gmail account to register. Make sure the spelling of the email address is correct and that all information is updated, accurate, and reliable. 

4. You will receive an email regarding your registration. Click on the link provided on the email.

5. Log-in using your registered email and password. 

6. Once logged-in, hover over the “K-12 Admission” button in the menu bar, then, hover over the “K-12 Admission Assistant” and click “Application.” 

7. Fill-out all the fields with your primary information, personal details, and family details. Then, click “Next.” 

8. Kindly upload your Grade 11 report card only for the evaluation and deliberation of subjects.

9. Check the declaration and click “Finish.” 

10. Pay the application fee amounting to P200.00.

11. Wait for the confirmation email in your Gmail account and keep your lines open for updates on your application.
